STATE FORMS
REFERENCE

For every state and the District of Columbia: the owner exemption, exclusion, or elective-coverage document, the agency that issues it, how it is filed, the fee, when it takes effect, and whether it renews. Where a state has no universal owner-exemption filing, that is stated plainly — it does not mean the state has no coverage rules. Every row links to the official source and shows the date it was last checked.

HOW TO READ THIS

“No universal form” means the owner rule is mostly statutory or handled through the insurance policy, not that coverage is optional. Some rows describe electing into coverage rather than out of it. Always confirm the current rule with the issuing agency before you file or change coverage — an exempt owner has no workers' compensation benefit for their own injury.

District of Columbia

No universal form
Issuing body

No universal owner-exemption filing is published for the sole-proprietor path; D.C. DOES states sole proprietors are not covered while employers with one or more employees must obtain coverage for employees.
Fee
No separate filing fee verified
Note
D.C. does not use a general owner-exemption certificate comparable to Florida or Tennessee for the sole-proprietor rule. Construction-permit proof requirements remain separate, and LLC/corporate-officer treatment must be checked against the Act and current DOES guidance.
Effective
No exemption filing date applies to the sole-proprietor exclusion described by DOES; ordinary employer coverage duties begin when covered employees are present.
